Which flood zones require flood insurance?

Any zone that starts with A or V (AE, A, AH, AO, VE, V) is a high-risk Special Flood Hazard Area. In one of those zones with a federally backed mortgage, flood insurance is required by law. Zone X is moderate to minimal risk and not federally required, though many coastal owners carry it anyway.
